what if the usb modem is lost
can someone please tell the complete formalities and cost implications if the usb evdo card is lost ?

nothing, if the device was on rental, an FIR Copy (a police report will also do), and full payment of the privious device for getting a new device.
If the device was not on rent, police report copy, and purchace reciept, and a new purchase or a new rental. at any cost, u r not going to get a replacement card or any thing, u will have to pay twice if device on rent and is already purchased, then u will have to pay the device cost again...
